Application Forms

Please download the appropriate application below.  All grants require completion of a RAC Data Collection Form (also below), which may be filled out online then printed.  If any file does not open, please email us to have it forwarded to you.

Potential applicants who need other grant application forms not listed here, or if needed in a different electronic format, should call the office at 507-281-4848.

Proposal Drafting Sessions

Applicants are encouraged to contact the office to sign up for a proposal drafting session during the month prior to the deadline, or to discuss ideas for their proposals if space is not available.  Although optional for most applicants, this is required for first-time Arts & Cultural Heritage applicants; failure to do so without previous approval of the SEMAC director will result in proposals being declared ineligible.  See the Calendar page for currently-available sessions.

Proposal drafting sessions are conducted by Deb Ward, MA, CFRE. A professional grant writer for over twenty years, Ms. Ward is a nationally recognized proposal writing consultant for education and non-profit clients. She has her own grants consulting business, Ward and Associates. Deb provides grant seeking and proposal writing workshops across the U.S. and has presented at local, state, national and international conferences. Currently she is a member of the Gundersen Lutheran Medical Foundation Development staff. She is the author of two grants-related books: Effective Grants Management and Writing Grant Proposals that Win, 4th edition.
